A Safe Table is a forum conducted under the federal law
establishing PSOs such as CHPSO, at which health care
professionals convene and have an open dialogue about patient
safety and quality issues. Frank and transparent discussion are
encouraged in this confidential and legally privileged setting.
Because this is an entirely protected forum, all information
shared will remain confidential and participants sign a privacy
agreement upon arrival at the meeting.
Safe Table Forums are currently being formed regionally, and once
a forum is created, meetings will be scheduled quarterly. If you
have interest in participating in a forum in your region, please

CHPSO will host safe table meetings at their offices or at hospitals. Children’s hospitals and pediatric units convene via teleconference on a quarterly basis, as do rural hospitals. A preliminary schedule is as follows:
