Overview

These “post-acute” services support a patient’s continued recovery from illness  and play a crucial role in patient care and recovery.  Additionally, as the health care delivery system continues to shift toward population health management, acute care providers will benefit from a better understanding of post-acute care’s role in the health care delivery system. 

This webinar series is designed to provide a foundation for acute care hospital leadership teams to better understand post-acute care services, as well as how to partner or engage directly with post-acute care providers. Join us for this three-webinar series and learn how to maximize post-acute care services and improve patient outcomes. 

The series is based on Productive Partners: Hospitals and the Continuum of Care Guidebook developed by Premier Performance Partners in partnership with the California Hospital Association. At the conclusion of the series, both a printed and electronic copy of the guidebook will be sent to all participants.

Recommended for: Post-acute professionals that work in long-term acute care facilities, skilled-nursing facilities, medical rehabilitation and home health services, including CEOs, COOs, CFOs, CMOs and CNOs.

Faculty

Andy Edeburn  is a Principal with Premier, Inc., with nearly 20 years of healthcare consulting experience, specializing in acute, primary, post-acute, and senior care services. He is a nationally recognized expert on aging services and post-acute care. His areas of expertise include strategic planning, acute/post-acute integration, provider network development, and managed care. Mr. Edeburn guides organizations through their strategic thinking and planning around acute and post-acute partnerships, new program and facility development and redevelopment efforts, and establishing value- and outcome-oriented relationships as organizations transition from the fee-for-service environment.

Donna Cella is a Director with Premier’s Clinical Transformation team and is a subject matter specialist in care management program assessment and redesign, clinical transformation strategy, and post-acute care. Over her nearly 30-year healthcare career, she has worked in complex/critical care, emergency/trauma services, elder care and clinical research. Ms. Cella is an accomplished facilitator who works closely with clinicians, leaders and aligned stakeholders to evaluate and adapt current models of clinical service and care management to address shifting perspectives around healthcare performance and payment.

Pat Blaisdell provides membership support and advocacy for hospital-based medical rehabilitation and skilled-nursing services, policy analysis and interpretation, communication with regulatory bodies and third-party payers, and planning and implementation of educational programs. Ms. Blaisdell has more than 25 years of experience in hospital and health care management in acute and post-acute settings. She has particular expertise in clinical operations and reimbursement across the post-acute continuum of care and is a Fellow of the American College of Healthcare Executives.

CEs

Participants must attest to participation in all three webinars to receive Continuing Education (CE) units.  If you miss a webinar, you may review the recorded version then fill out the evaluation to attain the CE credit. 

Health Care Executives — CHA is authorized to award 1.5 hours of pre-approved ACHE Qualified Education credit per webinar (or 6.0 hours for the full series) toward advancement, or recertification, in the American College of Healthcare Executives. Participants in this program who wish to have the continuing education hours applied toward ACHE Qualified Education credit must self-report their participation. To self-report, participants must log into their MyACHE account and select ACHE Qualified Education Credit.

Nursing — Provider approved by the California Board of Registered Nursing, CEP 11924, for 1.8 Contact Hours.

Nursing Home Administrators — Application has been made to the
State of California Department of Public Health to award nursing home
administrators continuing education for this program.

Rehabilitation Professionals — This program may meet continuing education requirements for Occupational Therapy, Physical Therapy and Speech/Language Pathology. Please request a Certificate of Attendance to submit to your professional organization.

Important note: While more than one individual from your organization may listen to the webinars, CEs will only be awarded to the registrant.

Tuition

Members*:  $90 for all 3 webinars
Multiple staff can participate from one location for one tuition fee.

This CHA members-only webinar series is specially priced at $90 for all three webinars and includes access to the webinar recordings and a printed copy of the Productive Partners: Hospitals and the Continuum of Care Guidebook to be mailed at the conclusion of the series. Only one registration needed to participate in all three webinars.

*Members are CHA member hospitals, CHA associate members and government agencies. 

Can’t make all the webinars? Catching up is easy. Every webinar will be recorded and available to view prior to the next event.

Cancellation Policy/Late Payment: A $35 nonrefundable processing fee will be retained for each cancellation. Cancellations must be made in writing seven or more days prior to the scheduled session and emailed to education@calhospital.org. No refunds will be made after these dates. Substitutions are encouraged. Please note that payment is due on or before program. Payments not received by the program date may be subject to a 10% late fee. In the unlikely event the program is cancelled, CHA will fully refund paid participants within 30 days.
