Patricia Quigley,
PhD, MPH, ARNP, CRRN, FAAN, FAANP, Nurse Consultant, Retired Associate
Director, VISN 8 Patient Safety Center of Inquiry, is both a Clinical Nurse Specialist and a Nurse Practitioner in Rehabilitation.

As Associate Chief of Nursing for Research, she was a funded researcher with the Research Center of Excellence:  Maximizing Rehabilitation Outcomes, jointly funding by HSR&D and RR&D. Her contributions to patient safety, nursing and rehabilitation are evident at a national level – with emphasis on clinical practice innovations designed to promote elders’ independence and safety. She is nationally known for her program of research in patient safety, particularly in fall prevention. The falls program research agenda continues to drive research efforts across health services and rehabilitation researchers.

Who should attend this Safe Table Forum:

Patient Safety, Quality, Risk Management, Clinical Informatics, all nursing units, same day surgery units/centers, and anyone from your organization who is interested in the topic.

What is a Safe Table?

Safe Table discussions are designed Safe-Tables are designed to empower providers to engage in robust, meaningful patient safety and quality improvement activities. These discussions are driven by attendee participation. Attendees may choose to present a case or simply participate in the discussion by sharing their thoughts, feedback, and suggestions. Attendees wishing to share a case should indicate this during registration and a member of the CHPSO team will reach out to you to assist you with case preparation. Sharing cases spontaneously during the meeting is also encouraged, time permitting. Attendees may present with or without slides. The cases may be brief, 3-5 minutes, or longer if the complexity of the issue warrants additional time. Cases generally involve a situation in which there are systematic concerns or those for which the organization may be looking for ways to address the matter or obtain advice.

CE Notice:

HQI is an approved continuing education (CE) provider by the California Board of Registered Nursing and will provide CHPSO members an opportunity to earn CEs. Provider Number CEP16793 for 1.0 contact hour.

HQI does not currently offer ACPE approved Pharmacy CE, but the State of California will accept CE from courses which have been approved for continuing education by the Medical Board of California, the California Board of Podiatric Medicine, the California Board of Registered Nursing or the Dental Board of California towards their California continuing education requirement (Section 1732.2(b) of the California Code of Regulations). Pharmacists need to keep track of CE from these sources on their own as they are not submitted to NAPB.

At the conclusion of the safe table meeting, participants will be able to:

  • Describe information about best practices relative to patient safety.
  • Discuss challenges in a frank, open discussion with other hospitals.
  • Describe lessons learned from other organizations as it could relate to the work in the participant’s hospital.
