Human factors play a key role in safety culture and in accident
causation. Effective safety management is a crucial determinant
of the safety culture of the organization.
Safety culture is the ways in which safety is managed in the
workplace, and reflects the attitudes, beliefs, perceptions and
values that employees share in relation to safety. In other words
“the way we do safety around here.”
“Transparency—the free, uninhibited sharing of information—is
probably the most important single attribute of a culture of
safety. In complex, tightly coupled systems like healthcare,
transparency is a precondition to safety. Its absence inhibits
learning from mistakes, distorts collegiality and erodes patient
trust.” (Leape, et al. 2009)
