CHPSO patient safety alert supplement—H&P Industries and Triad Shut Down

US Marshals raided H&P Industries, which makes alcohol wipes and other products distributed by Triad Group. This company was the source of several recent large-scale recalls and CHPSO Patient Safety Alerts. The manufacturer ceased production after yesterday's raid. Below is a partial listing of the recalled items. The latest FDA inspection report lists several other products of concern, including hemorrhoidal suppositories, benzalkonium swabsticks and psyllium husk products.

The FDA released a press release explaining the reasons for the raid: “We took this action to stop Triad from continuing to distribute products, which may pose a risk to public health.”

Important note: manufacturers of many of the recalled kits are noting that the medication or device is not defective, and that it can be used by discarding the recalled prep pad and using an alternative prep method. Please check the recall notice for each item for the specific manufacturer recommendations.

This company, a major supplier for many brands, appears to have had systemic issues that may affect more products than those already recalled. We recommend that organizations work closely with their suppliers to identify Triad or H&P products, including other manufacturers’ kits containing the affected prep products.
