Speakers:
James Bagian, MD, PE, is Director of the Center for Healthcare Engineering and Patient Safety at the University of Michigan. Dr. Bagian, a former NASA astronaut, was the founding director of the VA National Center for Patient Safety. His unique knowledge of engineering, medicine and aviation has made him an innovator in preventing adverse medical events.

Allan S. Frankel, MD, is Co-Chief Medical Officer at Pascal Metrics, which applies software-driven data science to improving patient safety. Dr. Frankel is senior faculty at the Brigham and Women’s Hospital Patient Safety Center of Excellence in Boston and the Institute for Healthcare Improvement. He has advised clinical and hospital leaders worldwide.

Dan Ford, MBA, FACHE, is Vice President with Furst Group, a healthcare executive search firm, and an ardent patient safety advocate. After his wife suffered permanent brain damage/ short term memory loss due to multiple medical errors, Dan has worked to create positive change, through national and international voluntary patient safety and quality committee participation, patient safety presentations, workshops and recruitment of executives who share his passion.

C. Duane Dauner, is President/ CEO of the California Hospital Association, which represents more than 400 hospitals and health systems statewide through leadership and advocacy. Dedicated to making patient safety a priority, he is a nationally recognized leader committed to changing the shape of health care for the better.

Rory Jaffe, MD, MBA, is Executive Director of CHPSO. Prior to heading up CHPSO, Dr. Jaffe was Executive Director of Medical Services for the University of California system, where he served as the senior physician in the system, overseeing quality of care at the five academic health systems and the ten student health centers.
